Use of Photography/Video Consent
Photographs or videos may be taken of my child participating in matches. They may be posted on the club website and used in other promotional material including affiliated league websites, social media and local media publications where appropriate.
Personal data consent
We require consent to process any data on your child’s medical conditions, dietary requirements or allergies.
The processing will be; providing this information to any coaches involved in your child’s activities in the club; providing it to any medical practitioners, as required, in your absence; and providing it to the league they are registered with when requested.
This processing is concerned with your child’s safety but requires us to seek your consent to use it in this way.
The club has a Privacy Notice to understand how we use all data provided.
Parent / carer declaration
I agree to my child’s participation in all football activities, including league, cups and tournaments.
I acknowledge the need for my child to behave responsibly at all times.
I will ensure that my child normally attends training/matches/tournaments as notified to me. I will provide notice of any absence.
I agree to my child receiving medication as instructed and any emergency dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present.
I have read and agree to be bound by the club’s code of conduct. I certify that the above information is correct and I consent to the information I have provided on this form being used by the Club for any purposes under the GDPR Act of 2018
I also certify that I do not have any outstanding liabilities with any of the above clubs I have previously been registered with. I understand that failure to disclose such information or make any false statement will render the registration invalid and liable to sanction.
